Paddock Tool Co. makes some quality bandsaw guides.
I've got Carter 500s on my 20" saw and I dislike them. They use the dinky 6200 size bearings, the same size as the home shop 14" band saws use. Those have to be replaced every few years. Actually I'll pop the seals (from the outside, not the inside), clean them, re-lube (40% volume) and get a few more years before they start screaming again.
The 6203 size was used on the venerable Delta 28-350 20" band saw as a backup bearing . Those would historically last 20-30 years in a school shop environment.
When looking for guides using ball bearings, be vigilant about the size bearings used.
