A design document (template)?

For those of you who have made custom built designs for a client - do you use a document template that details dimensions, finishes, and/or everything else?
Care to share the basic ideas?

It has been awhile - I used to draw a sketch, a rough 3d view, as the client described what they wanted. I would add details such as paint vs stain, wood type, etc. as we talked. Before I left the meeting I would write down a estimate plus or minus 25%, to make sure the client was onboard. No sense going farther if you are competing against Ikea.
