I'll echo Tod's comment and suggest that although expensive, drawer slides can be turned on their sides (so that they are flatter than usual) and may well work fine for you in this application. You'll either want full extension slides that are designed to keep the drawer from sliding off the end or incorporate some sort of stopping mechanism.

The biggest reason for thicker than 1" slides on most tables is that they are also supporting the table top. If they are merely guiding the top's motion and there is some other support available then you should be OK simply reducing the dimension of the the design you posted.

On a side note my parents have a really neat table that sits on two pedestals with a tray between that hold the two leaves of the table, and the slides have a steel rack and pinion on them so that when you pull on one end both ends move - this makes it really easy to open the table up for adding or removing leaves.
