My trailer coupler came off the hitch ball

Assuming you have a coupler like the first one DrBob posted are you using a safety pin like this?

Please post a photo of your coupler. There are different styles of 2” couplers and the root cause may vary between them.

I've pulled a lot of trailers and, knock on wood, have never had this happen thankfully. I always use a padlock to ensure that the mechanism doesn't come loose when hitting bumps etc. Always make sure to keep slightly more weight in front of the trailer axle to keep pressure down on the trailer tongue/hitch. It sounds to me like either the ball is too small, or the clamp inside the hitch is warn/broken/warped.

I'll echo what Scott said. Something is not adding up. My first thought was you had a 1 7/8 ball with a 2 in hitch but that was ruled out. second was the nut underneath the hitch that adjusts the locking mechanism needed tightening. You should be able to tighten that nut until you can barely get the hitch to go over the ball even with the locking latch open.

The hitch is adjustable. Lift up and down on the trail tongue, there should be minimal movement. If it feels loose, there is a nut under the coupler that can be tightened. You also should always use a locking pin so the coupler doesn't pop open.

In the old lever type coupler I had, there is a cross pin and there was a groove worn into it. I discovered this after the bolt was tightened as far as it could go and coupler still can loose when I raised the trailer.

It’s just not a good coupler for a large trailer.
