I checked my other router both brush caps were loose. Fortunately this one didn't fly too far away.

Nope. I have two PC690s and neither has done this. But the variable speed failed on one of them. So I just bypassed it and it is now single speed. Still works fine. I have four bases for the two motors.

I wondered if it hit you in the head when it popped out? Those routers are really turning fast another reason I never use 1/4 inch bits unless the speed is cut back. Glad it was just a repair on the tool.

Yes At work...the guys rarely check their equipment.... this is not all that uncommon. There is a repair kit that is pretty cheap. To buy the brushes is like 12-15 bucks and the caps are I think 5-6 bucks.
