Fall 2009 Raffle Mail-in Form 2019-04-16

This form may be used for entry into the Fall 2009 Raffle. It must be filled out completely (print please) and mailed with a check for tickets ($5 per chance) to the address shown on the form. For information, please send any questions to raffleinfo@ncwoodworker.net. All entries must be received by 6:00 PM on the Raffle Closing Date of November 16, 2009. Only one copy of the form needs to be included with your check.
